This is an amendment to the original contract to facilitate strategic planning, identify and invite key stakeholder groups, develop and administer an assets inventory/survey of current practices and policies associated with wellness, nutrition, physical activity, food and wellness within these settings: schools, workplaces, communities and healthcare for the following Nevada counties: Douglas, Lyon, Storey and Carson City. This amendment increases the maximum amount from $9,600 to $27,600 due to increased wellness-initiative activities in the school districts. |
