This is the fourth amendment to the original contract which provides in-home care services for frail elderly clients of the Community Services Options Program (COPE), the Title XX Homemaker Program and clients of the Personal Assistance Program (PAS) for persons with disabling conditions. This amendment extends the termination date from May 31, 2012 to November 30, 2012 and increases the maximum amount from $950,000 to $977,000. The term extension will allow Aging and Disability Services Division time to implement Provider Agreements or an RFQ process for future contracts with providers. |
